According to Florida state law a physician order is required for you to proceed with Medical Nutrition Therapy. Please contact your physician to obtain an order. If no order is provided at the initial appointment Paulette will reach out to your physician.
Paulette's client intake process includes gathering detailed information upfront. This process allows her to individualize and personalize your visit. This information allows her to prepare appropriately so your visit can be solution-focused. Please print the forms and complete them.
